(1) Which version of Photoshop do you have? I can help you if it's 6-CS, otherwise I can't be of much help.
(2) Normally if you're trying to save an icon, you want it to be one of the following types: PNG, JPG or GIF. GIF is for animations.
Look for a menu called Save For Web in Photoshop, or if it's animated, Save Optimized As in ImageReady. Save For Web *might* be under the Export menu (File/Export) if you have an older version of Photoshop.
What keeps coming out incorrectly when you view it? It might have to do more with the image viewer or settings of your picture than your actual program or icon...
